Form 4 Portal Privacy Notice
This portal collects representative name, designation, official email address, mobile number and facility information to create and administer Form 4 accounts, connect representatives to facilities, communicate filing updates, maintain audit trails and support TGPCB review.
Access and use
Authorised portal administrators, the concerned Regional Office and authorised State Office personnel may access information for registration validation, Form 4 administration, compliance review, support, security and reporting.
Corrections and support
Account and facility corrections must be requested through the portal helpdesk or concerned Regional Office. The production portal administrator must replace this paragraph with the approved helpdesk, grievance, retention and breach-response contacts before public launch.
Security
Passwords are handled by WordPress authentication and are not stored by this module in readable form. Portal actions relevant to account and facility administration may be recorded in an audit trail.
